Integrated health care delivery models for improving patient-centered care

Private sector health care providers along with federal and state governments are leading efforts to migrate away from fragmented fee-for-service health care towards more integrated care delivery models that improve coordinated care by aligning incentives with outcomes.  A variety of value-based care delivery models are gaining traction across the public and private marketplace- from patient-centered medical homes and ACOs to Medicare Advantage plans. By coordinating the various facets of the complicated health care system, these models promote preventative care and efficiently address the needs of individuals with multiple health care needs.

On March 16, Governance Studies at Brookings will host a discussion to examine the complexities of fragmented health care delivery and the value of more coordinated care provision. A panel of experts will explore potential benefits and barriers to expanding patient-centered care delivery approaches across the health care system. 

After the program, panelists will take audience questions.
