Diabetes care is a major public health challenge that affects 10 percent of Americans and nearly 350 million individuals worldwide, with diagnoses expected to increase significantly over the next several decades. Failure to address basic risk factors, appropriately manage those affected, engage patients in self-management, and coordinate care across settings, will only lead to worse outcomes and higher health care costs. This unsustainable trajectory has reinforced the urgent need for innovative care models across the globe that leverage new technologies and value-based payment models. Yet many obstacles stand in the way for providers and health care institutions. 

On April 7, the Richard Merkin Initiative for Payment Reform and Clinical Leadership convened international leaders from Spain, India, Mexico and the U.S to discuss new research and real-world application of leading global diabetes and chronic disease management programs. 

The event featured five case studies highlighting each country’s work and included perspectives from providers, payers and other health policy experts:
